Operator has not scheduled and obtained a fire inspection within 12 months of the previous inspection. Operator did not submit the original approved report to DCDEE within one week of the inspection visit on a form provided by the Division. A fire inspection was conducted on 10-11-2023, however the fire inspection prior to the new inspection was dated 6-27-2023.

Materials used for starting fires, such as matches, lighters and accelerants were not kept in locked storage. In the gym area which is used by the children on days that they cannot go outside due to weather, there were two lighters being stored on a cabinet near the doorway leading down the hallway.

In space 105 there were a package of batteries being stored on top of the cubbies.

Shelter-in-place or lockdown drills were not practiced every three months and/or drill record was incomplete. A shelter-in-place drill was conducted on 06-05-2023, however a lockdown drill was not conducted until 10-04-2023 which exceeds the three month time frame.
