Meals/snacks did not comply with the Meal Patterns for Children in Child Care Programs. Children was able to to choose from two different lunch menu. One (1) choice was Chicken, biscuit and gravy, scrambled eggs, yogurt, spiced apples and milk. The choice was PB & J, french fries, spiced apples, yogurt and milk. For the children who chose choice one (1), cherry tomatoes were on the menu but they were not served. No substitution for vegetables were served.

Departure information and parent's initials are missing on daily attendance sheet. In group #2, departure time for three (3) children and four (4) initials are missing for Monday, 10/24/22. One (1) child's departure time and six (6) initials are missing for Tuesday, 10/25/22. seven (7) initials are missing for Wednesday, 10/26/22. Four (4) departure time and five (5) initials are missing for Thursday, 10/27/22. In group #1, one (1) student's departure time was missing and one (1) initial was missing for Wednesday, 10/26/22.

Children's Benadryl in group #2 was stored with Epi-pen in an emergency backpack. Staff medication, including Excedrin Extra Strength and MiraLAX was found in a storage with broken lock.

The courtyard playground capacity is twenty-three (23) children per seventy-five (75) square per feet per child. There were twenty-nine (29) children on the playground during outdoor time.
