Correction Required: Toys and objects with a diameter of less than 1 inch, objects with removable parts that have a diameter of less than 1 inch, plastic bags and styrofoam objects may not be accessible to children who are still placing objects in their mouths.

Noncompliance Area: The rest equipment used by three of the six enrolled children were not labeled for use by these specific children.

Correction Required: Individual, clean, age-appropriate rest equipment shall be provided for preschool, toddler and infant children as agreed between the child's parent and the operator. The rest equipment shall be labeled for the use of a specific child and used only by the specified child.

Noncompliance Area: There were no child service reports completed, and a copy given to the parent within the last 6 months, on file for children #3 and #4.

Correction Required: An agreement shall specify the services to be provided to the family and the child, including the Department's approved form to provide information to the family about the child's growth and development in the context of the services being provided. the operator shall complete and update the form and provide a copy to the family in accordance with the updates regarding emergency contact information at 3290.124(e).

Noncompliance Area: The emergency contact forms on file for children #1 and #2 did not include the health insurance policy number.

Correction Required: Emergency contact information must include health insurance coverage and policy number for a child under a family policy or Medical Assistance benefits, if applicable.

Noncompliance Area: The emergency contact forms and agreements on file for children #3, #4 and #5 were not reviewed and updated by a parent in writing in the last 6 months.

Correction Required: The parent shall update in writing emergency contact information once in a 6-month period or as soon as there is a change in the informationA parent is required to review and update the emergency contact information and the financial agreement at least once in a 6-month period or as soon as there is a change in the information.

Noncompliance Area: There were no health assessments on file for children #1 and #2, both enrolled at the facility for more than 60 days.

Correction Required: The operator shall require the parent of an enrolled child, including a child, a foster child and a relative of an operator or a facility person, to provide an initial health report no later than 60 days following the first day of attendance at the facility.

Noncompliance Area: The immunization record on file for child #1, age 2 did not indicate the child had sufficient doses of the DTAP, pneumococcal, and Hepatitis A vaccines as recommended by the ACIP. The immunization record on file for child #2, age 5, did not indicate the child had sufficient doses of the Hepatitis B, pneumococcal, and Hepatitis A vaccines as recommended by the ACIP. There was no documentation of of an influenza vaccine - recommended by the ACIP annually - from within the last12 months on file for children #1, #2, and #3.

Correction Required: The facility shall require the parent to provide updated written verification from a physician, physician's assistant, CRNP, the Department of Health or a local health department of ongoing vaccines administered to an infant, toddler or preschool child in accordance with the schedule recommended by the ACIP.

Noncompliance Area: On the date of the inspection it was observed that the operator did not have a fire extinguisher located in the cooking area (kitchen). This condition poses a threat to the health and safety of children.

Correction Required: Conditions at the facility may not pose a threat to the health or safety of the children. The operator will demonstrate an acceptable portable fire extinguisher to an agent of the department.

Noncompliance Area: The regulations under which the facility is certified were not posted.

Correction Required: The facility's current certificate of registration and a copy of the applicable regulations under which the facility is certified shall be posted in a conspicuous location used by parents, with instructions for contacting the appropriate regional day care office posted at the same location

Noncompliance Area: There was an uncovered outlet on the wall in the basement child care space.

Correction Required: Protective receptacle covers shall be placed in electrical outlets accessible to children 5 years of age or younger.

Noncompliance Area: There were cleaning supplies in an unlocked cabinet under the sink in the kitchen, in reach of children. There was a container of Clorox wipes on the changing table in the basement child care space, in reach of children in care.

Correction Required: Cleaning materials and other toxic materials shall be stored in an original labeled container or in a container that specifies the content. Toxics shall be kept in a locked area or in an area inaccessible to children, and shall be stored away from food, food preparation areas and child care spaces.

Noncompliance Area: There were no tweezers in the first aid kit.

Correction Required: A first-aid kit must contain the following: soap, an assortment of adhesive bandages, sterile gauze pads, tweezers, tape, scissors and disposable, nonporous gloves.

Noncompliance Area: The evacuation routes to be used in a fire were not posted.

Correction Required: Fire evacuation plans shall be developed and posted.
