We covered bolts with a yellow cap and the other one we wrapped Gorilla tape around it. In the future we will use a checklist we just received from our Program Certification Representative and we will use the checklist to follow through on a weekly basis. We will split up the areas and complete the entire checklist weekly. In the future we will immediately fix all toys, play equipment and other indoor our outdoor equipment used by the children. We will clean and make sure all equipment are free of rough edges, sharp corners, pinch or crush points or splinters.

The supplies were removed on 2/24/2020 from the cabinet and stored in a locked cabinet. The lock was fixed at the end of the day on 2/24/20. In the futre the teacher will complete a daily checklist to look for any items that have to keep out of reach of children including plastic bags and make sure all of these items are in a locked cabinet.

In the video, inspector observed staff 2 grabbing child 1, pushing child 1's face to the floor, flinging child 1 to the floor, holding child 1 down on the floor and restraining child 1. Staff member#2 and co-director will attend the existing provider orientation on November 19, 2019. When a child is throwing objects in classroom, hitting teacher or other students, we will evacuate room and leave one staff if not two in the room to monitor child's safety. No staff will restrain child while he/she is angered and throwing objects and hitting staff. Staff will remain in room until child has calmed down and parent has been called to take child home. We also changed our seclusion policy and will disenroll and child who knowingly and intentionally hurts a peer or teacher or damages property. No form of physical punishment or restraint will be used by any staff.

In the future if a staff member is getting upset by a parent, director will remove staff member from the room to avoid harsh language being used. Noncompliance Area: On 8.30.19 a complaint inspection was conducted. Inspector walked through the infant room. The curtains were drawn not allowing any natural light into the room. The room was dark and inspector was unable to see the baby's sleeping in the cribs. Inspector had to lean into the cribs of the babies sleeping in order to see the baby's face and if the baby was breathing. Inspector observed staff person 5 in the kitchen making a bottle and was unable to assess the children in the cribs from the kitchen in the dark. Staff person 5 was approximately 20-25 feet away from the farthest baby in the crib sleeping. Staff person 6 was sitting closer to the closed curtains in a rocking chair approximately10-15 feet away from the cribs feeding a baby. Inspector provided technical assistance to the owners husband on supervision.

Correction Required: Rooms, hallways, stairways, outside steps, porches and ramps shall be lighted by artificial or natural light.Children on the facility premises and on facility excursions off the premises shall be supervised by a staff person at all times. Outdoor play space used by the facility is considered part of the facility premises. Children must be supervised at all times. The legal entity must arrange for all facility staff to receive three hours of PQAS approved in-person training regarding supervision of children. It does not matter if the PQAS approved training is through the ELRC. The legal entity must receive DHS approval of the training content prior to scheduling the training.

Staff #1 is in contact with individual who has spoken to our program certificate representative to have all staff have in-person training dealing with supervision of children. When a bottle needs to be made during nap, the head teacher will make sure a third person comes into the room to help supervise the infants. Also, head teacher in each room will make sure all curtains are opened. If it is a cloudy day, lights will be on in room to insure that all children can be seen during nap. Children will be supervised at all times.
