|Findings: After a review of 2 staff files, it was determined the following one was lacking: Staff 1 (3/20/2017) documentation of a central registry affidavit.|
|Findings: After a review of 10 children's Emergency, Information and Immunization Record cards, it was determined the following was lacking: Child 1: Health care provider's name and phone number. Child 2: Health care provider's name and phone number; the name of the person to call first in case of sudden illness/injury.|
|Findings: There are gas appliances in the cafeteria kitchen. The main activity area for the enrolled children is the cafeteria. The was no gas inspection available at the time of the inspection.|
|Findings: After a review of 2 staff files, it was determined the following one file was lacking: Staff 2 (start date 9/6/2016) had documentation of 11 months of the required 2 years of child care experience to qualify as the director.|
|Findings: After a review of 2 staff files, it was determined the following one file was lacking: Staff 1 (start date 3/20/2017) was listed as the director's designee. Staff 1's file lacked documentation of 38 hours of required training, and 1 year of child care experience.|
|Findings: Upon arrival at the facility the surveyor observed the staff to child ratio to be 1:8. The director stated the second staff member was sent to another site because the other site had a lot of kids.|
|Findings: The Department put the licensee on notice in November 2015 that the controlling persons needed to be updated. As of the time of the inspection the Department has not received updated information.|
|Findings: The following items were not posted: name of the facility director, the name of individual to to act on the director's behalf, fees and refunds, and the menu.|
|Findings: Upon arrival at the facility there were 10 children and one staff member.|
|Findings: The first aid kit lacked gauze rolls.|
