Initial Comments: There were no deficiencies observed at the time of the annual compliance inspection conducted on 08-25-2025. 2 of 2 fingerprint clearance cards were verified to be valid through the DPS website during the time of the inspection. The Compliance Officer provided the facility with a paper copy of the Notice of Inspection Rights at the start of the inspection. The DES Contact Group size was in compliance at the time of the inspection. The following was discussed, but not limited to: 1. Storage of cleaning equipment. 2. Posted menu documentation. 3. Emergency card documentation. 4. Immediate access on site.

Initial Comments: There were no deficiencies observed at the time of the Initial Monitoring inspection conducted on October 21, 2024, subject to change pending programmatic review. Compliance Officer: Celeste Angulo A full inspection was not conducted at this time. 1 of 1 fingerprint clearance cards reviewed were verified through the DPS website during the inspection. A link to the Empower Survey was emailed to the facility director. The Emergency Disaster Form was completed during the inspection. The following items were discussed but not limited to: **Maintaining classroom roster in the current activity area **Criminal History Affidavit and Background Check to be completed before staff date of employment **Discussed completeness of EIIR cards **Discussed change of Director within the Portal

Initial Comments: There were no deficiencies observed at the time of the initial compliance inspection conducted on August 28, 2024, subject to change pending programmatic review. Compliance Officer Supervisor: Peggy Kraus Compliance Officer: Stacy Marchelli A complete inspection was conducted at this time. Seven fingerprint clearance cards were verified through the DPS website during the inspection. Technical assistance was given regarding the following but was not limited to: 1. The construction dust in the gym. 2. Criminal History affidavits shall include the facility's address and/or CDC. 3. Supervision shall be used when using the classroom's hand sanitizer dispensers.
