A paper copy of the Notice of Inspection Rights was given to the Facility Director at the time of the inspection. Please submit the Plan of Corrections via the LMS portal within 10 days of receipt of the Statement of Deficiencies. The Emergency Disaster Contact form was completed at the time of the inspection. The Empower Self-Evaluation invitation was emailed to the Provider. The DES Group Size was observed in compliance at the time of the inspection. The fingerprint clearance cards for 2 of 2 staff members were verified to be valid through the DPS website at the time of the inspection. During the exit interview, the following items were discussed but are not limited to: *Staff files will include a copy of the front and the back of the renewed fingerprint clearance card *Staff will complete a Criminal History Affidavit each time the fingerprint clearance card is renewed *24 hours of annual training will be documented *Emergency drills will be documented

The Empower Self-Evaluation invitation was sent at the time of the inspection. The DES Group Size was observed to be compliant at the time of the inspection. The fingerprint clearance cards for 2 of 2 staff members were verified to be valid through the DPS website at the time of the inspection. During the exit interview, the following items were discussed but are not limited to: *The Emergency, Information and Immunization Record (EIIR) card should have the name and contact telephone number of at least two individuals authorized by the child’s parent to collect the child from the facility in case of emergency, or if the child’s parent cannot be contacted. *If the EIIR card states the child is prescribed an Epi-Pen to treat a potential allergy and the Center is not provided an Epi-pen, then the parent should make a notation on the EIIR that they will not provide an Epi-pen.
