|Findings: Based on review of staff files Surveyor 2 noted the following: -Staff 1 was not able to document 12 or more hours of annual training as required. 10.5 hours of annual training were documented between August 05, 2018 and August 05, 2019. -Staff 2 was not able to document 12 or more hours of annual training as required. 10 hours of annual training were documented between May 12, 2019 and May 12, 2020.|
|Findings: The Surveyor reviewed ten Emergency, Information, and Immunization Record cards and observed the following: - The card for Child #1-4 did not contain the date of enrollment. - The card for Child #5 did not contain the date of enrollment or the physician's name or phone number. - The card for Child #6 did not contain the physician's name or phone number.|
|Findings: The Surveyor reviewed the children's sign in and out logs and observed the following: - A time in was not documented for Child #5 on 6/7/19. - A signature and time out were not documented for Child #5 on 6/18/19. - A signature and time out were not documented for Child #6 on 6/7/19. - A signature out was not documented for Child #7 on 6/20/19.|
|Findings: The Surveyor inspected the outdoor activity area and measured three inches of dirt and wood chips under and around the yellow slide.|
|Findings: The Surveyor requested to inspect the soiled diaper container and, per Provider, one is not used for diapering.|
|Findings: The Surveyor reviewed ten Emergency, Information, and Immunization Record cards and observed that the cards for Children #1 - #6 did not contain their dates of enrollment.|
