"United Health" was written in the field reserved for the physician's name. -The cards for Child #3 and Child #4 did not contain a physician's name or telephone number. Both fields were left blank.

Findings: During the inspection of the outdoor activity area, Surveyor 1 measured the fence enclosing the area to be 3 ft, 6 inches along the north side and 3 feet 10 inches along the east side.

Findings: The Team Leader noted that the fire extinguisher mounted in the kitchen did not have a tag to verify that it was current. Staff 1 reported that it had been recently purchased, but said that she did not have any documentation to provide.

Findings: The Team Leader observed a diaper changing pad on the diaper changing table that had a vinyl cover that was stitched along a seam. Therefore, it would not be possible to sanitize the diaper changing surface. Staff 1 brought out a newly purchased changing pad that was stitched in a quilted pattern and that had straps that would not allow for sanitizing the pad.
