Findings: Upon entry into the home the Surveyor observed that a smoke detector was chirping in the living room area.

Findings: The Surveyor inspected the diaper changing area and observed that the soiled diaper container was not lined with a bag.

Findings: Upon reviewing the First Aid Kit, Surveyor 1 noted that there were no gauze rolls in the kit. Staff 1 reported that there were none elsewhere in the home.

Findings: Upon inspection of the Group Home's mats used for children to nap, the Team Leader observed that two of the mats were torn and had exposed foam and therefore are no longer waterproof.

Findings: During the inspection of the outdoor activity area, Surveyor 1 measured the fence enclosing the area to be 3 ft, 6 inches along the north side and 3 feet 10 inches along the east side.

Findings: The Team Leader noted that the fire extinguisher mounted in the kitchen did not have a tag to verify that it was current. Staff 1 reported that it had been recently purchased, but said that she did not have any documentation to provide.

Findings: The Team Leader observed a diaper changing pad on the diaper changing table that had a vinyl cover that was stitched along a seam. Therefore, it would not be possible to sanitize the diaper changing surface. Staff 1 brought out a newly purchased changing pad that was stitched in a quilted pattern and that had straps that would not allow for sanitizing the pad.

Findings: Based on Surveyor and Team Leader's observation, review of facility documentation and information provided by the staff at the time of the inspection, it was determined that the dog at the group home lacked documentation of a current vaccination against rabies
