|Findings: Surveyor #1 observed a climbing structure with slides that was not on non dormant growing grass or 6" of resilient surface.|
|Findings: The Team Leader observed a surge protector that had outlets not covered that was accessible to enrolled children.|
|Findings: The Team Leader and Surveyor observed a container of Clorox wipes on the shelf that was accessible to enrolled children. The container was moved at the time of inspection.|

|Findings: a) Surveyor 2 observed that Staff 1 only had 8 annual training hours for employment year 2/9/18 - 2/9/19.|
|Findings: a) Surveyor 2 reviewed ten children's emergency records. The following information was missing: b) Child 1 - Dr. phone number. c) Child 2 - The name and contact telephone number of one additional individual authorized by the child ' s parent to collect the child from the child care group home or to be contacted if the child ' s parent cannot be contacted. d) Child 3 - Dr. phone number and the name of the individual to be contacted in case of injury or sudden illness of the child.|
|Findings: a) Surveyor 2 observed that the gas inspection expired on 9/12/19.|
|Findings: The written feeding instructions for Child #1 were observed to be incomplete by the Surveyor.|
|Findings: The Surveyor observed that the diaper changing table had plastic cover that was coming loose causing the table to not be able to be sanitized.|
|Findings: Team Leader and Surveyor observed dog feces in the outdoor activity area while children were present.|
