|Findings: The Surveyor reviewed the enrolled children's sign in and out records and found the following deficiencies: *Photos were taken Child 1 was not signed in on 3/14 and 3/15. Child 2 was not signed out on 3/11. Child 3 and 4 were not signed out on 3/12. Child 5 was not signed out on 3/1,3/7, 3/19 and 3/20.|
|Findings: The Team Leader observed sting relief medication in the first aid kit.|
|Findings: The Surveyor observed that the crib mattress was on the highest setting for Child 1, who was able to sit up. The Surveyor also observed Child 1 asleep in the crib with an amber necklace on, and a second child was observed to have a pacifier keeper while asleep in the crib.|
|Findings: The Surveyor observed that the infants in the bedroom were not being supervised. The Provider had the sound machine set on maximum level, the door was closed, and the television was on in the living room. The Provider could not hear the children and was not accountable for the children.|
|Findings: The Surveyor observed that a child was asleep in the living room on a mat that was not covered with a sheet.|
|Findings: The Surveyor observed that an infant child was asleep in the crib with a bottle in their mouth. The bottle contained a white liquid. The Provider did not have written parental permission to have a bottle in the crib.|
|Findings: The Team Leader observed that the posted menu was not dated, and did not specify the items to be served.|
|Findings: The Surveyor observed a bottle in an infants mouth asleep in the crib, and a bottle on the diapering surface. Neither bottle was marked with an identifier that is specific to that child.|
|Findings: The Surveyor observed food on the floor. The Surveyor observed a child eating a cheeze-it off the floor.|
|Findings: The Team Leader observed that the shade canopy in the back yard was shredded, and no shade was provided for the enrolled children.|
|Findings: The Surveyor observed that the annual gas inspection expired 3/5/2019.|
