|Findings: Based on review of required documents, as well as staff statements, the Surveyors determined that Staff 1 has not been signing in and out of the child care group home as required.|
|Findings: Based on review of staff files Surveyor 2 noted that Staff 1's file lacked documentation of 12 or more hours of annual training as required. There were zero hours of annual training documented between 09/10/2016 and 09/10/2017.|
|Findings: Based on review of Emergency, Information and Immunization Record (EIIR) cards Surveyor 1 noted the following: -Child 1's card did not document the child's gender, nor did it document the name of the individual to be contacted in case of injury or sudden illness of the child. Additionally, the card did not document the contact number of the child's physician as required. -Child 2's card did not document the date of enrollment as required.|
|Findings: Surveyor 1 observed the following items in the diaper changing area that were not labeled with a name or identifier: -Desitin diaper cream -A & D ointment|
|Findings: The Provider was not able to document Infant Feeding Instructions for an Infant, Child 4, that was present during the inspection.|
|Findings: The fire drill log provided during the inspection for January through May did not document that it was for 2018.|
|Findings: Based on review of diaper-changing documents Surveyor 2 noted that a log had not been started for the two diapered children, Child 3 and Child 4, present during the inspection.|
