Description: Clean and good repair

Noncompliance Area: In the Penguin Room, there was a small purple Magna-Tile and large pink Magna-Tile that were cracked posing a cutting hazard. In the Elephant Room, there was a blue bin behind the teacher's desk with Mr. Potato Head pieces that was cracked posing a pinch point or cutting hazard. In the butterfly room, in the play refrigerator was a blue bin with plastic toy fruit that was cracked posing a pinch point or cutting hazard. In the butterfly room there was a green bin with wooden blocks in the middle of the room next to the window on the 2 x 2 bookcase that had a piece missing which was exposing a sharp edge which poses a cutting hazard.

Correction Required: Toys, play equipment and other indoor and outdoor equipment used by the children shall be clean, in good repair and free from rough edges, sharp corners, pinch and crush points, splinters and exposed bolts.

Description: Small Toys and Objects

Noncompliance Area: There was a puppet in the Bunny room that had loose, stitched on beaded eyes and nose, smaller than 1 inch, that were loose and therefore a choking hazard to children still placing objects in their mouths.

Correction Required: Toys and objects with a diameter of less than 1 inch, objects with removable parts that have a diameter of less than 1 inch, plastic bags and styrofoam objects may not be accessible to children who are still placing objects in their mouths.

Noncompliance Area: There was a shelving unit in the Bunny room that had contact paper on the top surface that was peeling.

Correction Required: Furniture must be durable, safe, easily cleaned and appropriate for the child's size, age and special needs.

Noncompliance Area: The emergency contact form on file for Child #2, #6, and #8 does not include a complete address for all listed release persons.

Correction Required: Emergency contact information must include the name, address and telephone number of the individual designated by the parent to whom the child may be released.

Correction Required: The operator shall require the parent of an enrolled child, including a child, a foster child and a relative of an operator or a facility person, to provide an initial health report no later than 60 days following the first day of attendance at the facility. A child's record shall contain initial and subsequent health reports.

The most recent health assessment on file for Child #4, who is a preschool age child, is dated 10.2.24, more than 12 months ago. The most recent health assessment on file for Child #5, who is an older toddler age child, is dated 2.7.25, more than 12 months ago.

Correction Required: The operator shall require the parent to provide an updated health report at least every 12 months for an older toddler or preschool child. A child's record shall contain initial and subsequent health reports.

Noncompliance Area: There were 3 prescription medications labeled for use by specific children in the Butterfly room that were not in the original container.

Correction Required: A prescription or nonprescription medication may be accepted only in an original container. The medication shall remain in the container in which it was received.

Description: Name on bottle

Noncompliance Area: An unlabeled Albuterol container, reported by staff to be intended for specific children are lacking the name of the child for whom the medication for whom the medication is intended.

Correction Required: The label of a medication container shall identify the name of the medication and the name of the child for whom the medication is intended. Medication shall be administered to only the child whose name appears on the container.

Noncompliance Area: Diazepam, labeled for use by a specific child, in the Lion room, had an expiration date of 5.21.25, and was therefore not in accordance with the manufacturer's original label.

Correction Required: Medication shall be stored in accordance with the manufacturer's or health professional's instructions on the original label.

Description: Written consent

Noncompliance Area: In the Butterfly room there was an Epinephrine Pen labeled for use by a specific child that did not have written parental permission to administer, and 2 Albuterol inhalers labeled for use by specific children that did not have written parental permission to administer. There was an Albuterol inhaler in the Lion room labeled for use by a specific child which did not have written parental permission to administer.

Correction Required: A parent shall provide written consent for administration of medication or a special diet.

Regulation: 3270.134(a)

Description: Child's hands washed

Noncompliance Area: While observing a diaper change in the Bunny room, it was observed that the child's hands were not washed following the diaper change. The child was observed to be able-bodied and capable of walking to the sink.

Correction Required: A staff person shall ensure that a child's hands are washed before meals and snacks, after toileting and after being diapered.

Noncompliance Area: The Shaken Baby Policy submitted by the facility does not include language regarding the prevention and identification of child maltreatment.

Correction Required: A facility shall be operated in conformity with applicable Federal and State laws and regulations.State agencies whose regulations may relate to the operation of a facility include the Department of Environmental Resources, the Department of Labor and Industry, the Department of Health, the Department of Education and the Department of Transportation. Conditions at the facility may not pose a threat to the health or safety of the children.

The 2 most recent health assessments on file for Staff person #4 were dated 10.27.22 and 12.4.24, more than 24 months apart. The 2 most recent health assessments on file for Staff person #5 were dated 2.14.24 and 3.10.26, more than 24 months apart. The file for staff person #9 does not include documentation of a health assessment being completed within 12 months prior to date of hire (See LIS CODE SHEET). The health assessment on file for staff person #9 was dated 2.23.26.

Correction Required: A facility person providing direct care who comes into contact with the children or who works with food preparation shall have a health assessment conducted within 12 months prior to providing initial service in a child care setting and every 24 months thereafter. A health assessment is valid for 24 months following the date of signature, if the person does not contract a communicable disease or develop a medical problem. A facility person's record shall include a written report of initial and subsequent health assessments, including the results of initial and subsequent tuberculin skin tests, x-rays or other medical documentation necessary to confirm freedom from communicable tuberculosis.
