Description: Welcome to Eliada. Founded in 1903, we are a non-profit agency dedicated to our mission of helping children succeed. Located on a historic 200-acre campus in the mountains of Western North Carolina, we serve children and families through a diverse spectrum of programs and services, including residential care, child development services, foster care, community support services, therapeutic recreation, and many others.

Children were not adequately supervised at all times. On 3/6/23 around 3:30 pm, a group of two-to-three-year children transitioned from classroom to the playground. One teacher took eight (8) children to the playground. The other teacher remained in the classroom with three (3) children and completed diapering and snack. Upon transitioning to the playground, two (2) children were taken to the playground and one (1) child was left in the classroom unsupervised. The child was in the classroom alone for approximately six (6) minutes.

The staff/child ratios for children, two years and older, during naptime were not maintained by having at least one person in each room, visually supervising all children and the total number of required staff on the premises within calling distance of each room occupied by children. During rest time, two (2) teachers were present in the classroom space 14. One (1) teacher was working on a laptop and one (1) teacher was on a smart phone. One (1) teacher was present in classroom space 13. The one (1) teacher was observed to be looking at their phone during rest time.

The written feeding plan did not include the child's name, parent signature, and/or was not dated when received by the center. The infant feeding plans for children MC, MM, MB, and RE were not signed by the parent.

All equipment and furnishings were not in good repair. On the three playgrounds of the main building, rust was on various sections of the fences. Peeling paint was apparent on a Jeep structure on the PreK playground at the main building an on a metal bench on the infant playground at the main building.

Electrical outlets and power strips, not in use, which were located in space used by children did not have safety outlets or were not covered with safety plugs unless located behind furniture or equipment that cannot be moved by a child. Outlets by the stage in the gym of the main building were uncovered.

Electrical cords were accessible to infants and toddlers. In classroom space 12 of the Community building, the cord for the string lights was accessible to children.

Outdoor play area was not enclosed by fence with a minimum height of 4 feet. The top of the fence, less than six feet, was not free from protrusions. The fence surrounding the NC PreK classroom and School Age playground measured between 42.5 inches and 44.5 inches on the hill area of the playground. This was shown and discussed with Tonya Reed during the visit.
