Incident reports were not completed each time a child was injured, it did not include all the information required in rule, it was not signed by the parent and/or it was not maintained in the child's file. Five (5) incident reports were not signed by the staff member who completed the form and three (3) incident reports were not signed by the child’s parent or guardian for proof that the incident was reviewed with the parent and a copy was given.

Each child was not attended to in a nurturing and appropriate manner, or in keeping with the child's developmental needs. On September 26, 2024, a staff member took a four-year-old child by their forearm to guide them to a different area of the classroom. The child fell to the floor and the staff member did not release the child’s forearm and pulled the child from the floor to a standing position. As a result, the child sustained bruises/marks on the child’s forearm.
