In the hallway bathroom used by preschool children, there was an air freshener spray, a bottle of cleaner, and a lotion being stored in an unlocked cabinet under the sink. In space 2b, a bottle of disinfectant was being stored on top of the paper towel holder below five feet. In space 2a, a plug in air freshener with liquid was present in an outlet below 5 feet.

Plastic bags, materials that could be torn apart and toy parts small enough to be swallowed were accessible to children under three years of age. In space 1, a plastic bag was being used for storage in an unlocked cabinet under a sink accessible to the children.
