Noncompliance Area: In room C13, the preschool room, a cot was torn and the metal bar was exposed.

Correction Required: Individual, clean, age-appropriate rest equipment shall be provided for preschool, toddler and infant children as agreed between the child's parent and the operator. The rest equipment shall be labeled for the use of a specific child and used only by the specified child.

Description: Refrigerator

Noncompliance Area: The thermometer in the refrigerator in the young toddler room, room C17-C19 and Infant room read above 45° F. the temperatures were 50° F, 50° F and 54° F, respectively.

Correction Required: A facility shall have an operable, clean refrigerator used to store potentially hazardous foods. The refrigerator shall be capable of maintaining food at 45° F or below. An operating thermometer shall be placed in the refrigerator.

Noncompliance Area: The emergency contact form for child #3 was dated 2/1/24. The previous date of review was conducted on 6/5/23. This timeframe reflects longer than the required 6 month parent review.

Correction Required: The parent shall update in writing emergency contact information once in a 6-month period or as soon as there is a change in the information. A parent is required to review and update the emergency contact information and the financial agreement at least once in a 6-month period or as soon as there is a change in the information.

Description: Initial health report for infant dated no more than 3 months

Noncompliance Area: The initial health assessment for child #2 was dated 12/6/23. This health assessment was not dated within 3 months prior to the child's first day of attendance at the facility (see LIS code sheet for date of admission).

Correction Required: The initial health report for an infant must be dated no more than 3 months prior to the first day of attendance at the facility.

Regulation: 3270.135(b)

Description: Surfaces cleaned

Noncompliance Area: In the Infant and young toddler rooms, a cloth cover was on top of the changing table and prevented the table from being cleaned after each use with a sanitizing solution.

Correction Required: Diaper changing surfaces shall be cleaned after each use by wiping the surface with a sanitizing solution or by changing a pad or other surface covering.

Description: 2 hour diaper check

Noncompliance Area: In both infant rooms, the diapering logs for the children in care were not completed, therefore, this shows no indication that diapers are being checked every 2 hours.

Correction Required: A staff person shall check a child's diaper at least every 2 hours and whenever the child indicates discomfort or exhibits behavior that suggests a soiled diaper. A staff person shall change a child's diaper when the diaper is soiled.

Description: Menus

Noncompliance Area: There was no menu posted in the facility.

Correction Required: An operator shall conspicuously post the menu at least 1 week in advance or provide a menu to each family.

Noncompliance Area: In the infant rooms, a written statement giving the formula and feeding schedule for an infant was not obtained from the parent.

Correction Required: A written statement giving the formula and feeding schedule for an infant shall be obtained from the parent.

Description: Age and Training

Noncompliance Area: Staff person #1 and staff person #2 did not obtain an annual minimum of 12 clock hours of child care training during the 12/22-12/23 training period. The hours obtained were 8.5 and 4.5, respectively.

Correction Required: A staff person shall obtain an annual minimum of 12 clock hours of child care training.

Description: Building Surfaces

Noncompliance Area: In room B10-12, older toddler room, a microwave was stored on top of a mini fridge. The microwave was not secured to a surface and was able to be tipped over. In room C16 and Infant room, the changing tables were not secured to a surface and were wobbly.

Correction Required: Floors, walls, ceilings and other surfaces, including the facility's outdoor play space surfaces shall be kept clean, in good repair and free from visible hazards.

Regulation: 3270.77(a)

Description: No peeling paint or plaster

Noncompliance Area: Chipped paint was observed in the bathroom on the third floor.

Correction Required: Peeled or damaged paint or damaged plaster is not permitted on indoor or outdoor surfaces in the child care facility.
