Noncompliance Area: During an allocated unannounced inspection on 6/17/26 Cert rep observed Aquaphor (expiration 10/2024) and A& D ointment(expiration 10/2025), in Room 2.

Correction Required: Medication shall be stored in accordance with the manufacturer's or health professional's instructions on the original label.

Description: Emergency plan/Letter to parents

Noncompliance Area: During an allocated unannounced inspection on 6/17/26 the facility emergency plan and the letter to the parents regarding the emergency plan did not include information regarding the facilities lockdown procedures.

Correction Required: The facility shall have an emergency plan that provides for accommodations for shelter of children during an emergency including lock-down, shelter in place at the facility and shelter at locations away from the facility premises. The operator shall provide to the parent of each enrolled child a letter explaining the emergency procedures. The operator shall also provide to the parent of each enrolled child a letter explaining any subsequent update to the plan.

Description: Health and Safety Training

Noncompliance Area: During and allocated unannounced inspection on 6/17/26 Staff person #1 did not completed the following required pre- service training within 90 days of their date of hire (see LIS code sheet): Pediatric First Aid and CPR, and a complete PA Required Health and Safety Training. This is evidenced by by following documentation in Staff Person # 1's file: Pediatric First Aid and CPR training certificate dated 1/23/26, PA Required Health and Safety for Centers-Group Child Care Part 1 dated 4/8/26, and PA Required Health and Safety for Centers-Group Child Care Part 2 dated 4/11/26. Staff Person #1 was reported to work as staff between their 90th day of hire and 4/11/26. Staff Person #1 did not have any previous acceptable versions of the health and safety training completed.

Correction Required: Staff persons shall complete professional development within 90 days of hire as listed in subsections (f)1-10.

Description: Hypothetical locations

Noncompliance Area: During an allocated unannounced inspection on 6/17/26 review of the fire drill log showed that two consecutive fire drills(5/1/26 and 6/9/26) had the same hypothetical location of the fire ("lobby").

Correction Required: The Director or designated staff person who is responsible for compliance with this chapter ensure the hypothetical locations of the fire are rotated around the facility for each drill such that the hypothetical location is never the same for consecutive drills.

Description: 2 feet apart

Noncompliance Area: During a renewal inspection on 10/22/25 Cert rep observed 2 of the nap mats in the toddler area had less than two feet of space on three sides.

Correction Required: At least 2 feet of space is required on three sides of a bed, cot, crib or other rest equipment while the equipment is in use.

Description: Refrigerator

Noncompliance Area: During a renewal inspection on 10/22/25 Cert rep observed the thermometer in the refrigerator in Room 2 read 47° F.

Correction Required: A facility shall have an operable, clean refrigerator used to store potentially hazardous foods. The refrigerator shall be capable of maintaining food at 45° F or below. An operating thermometer shall be placed in the refrigerator.

Regulation: 3270.113(a)/3270.113(a)(1)

Description: Supervised at all times /Staff assigned to specific children

Noncompliance Area: During a renewal inspection on 10/22/25 Cert rep observed Child 1 in Room 2 with a service provider. Staff 4 who had Child 1 in their assigned group, was present in Room 2 however Staff 4 was not able to see and assess the child and service provider's interactions due to a tall room divider that blocked Staff 4's view.. Child's file did not have any documentation to support that the child was signed out or that parents had given consent for the child to be alone with the service provider while receiving these services. The child's IEP did not note the requirement for the child to be alone with the service provider and not supervised by facility staff.

Correction Required: Children on the facility premises and on facility excursions off the premises shall be supervised by a staff person at all times. Outdoor play space used by the facility is considered part of the facility premises. Each staff person shall be assigned the responsibility for supervision of specific children. The staff person shall know the names and whereabouts of the children in his assigned group. The staff person shall be physically present with the children in his group on the facility premises and on facility excursions off the facility premises. TIERED LIS: 1. Children must be supervised at all times. Each staff person shall be assigned the responsibility for supervision of specific children. This portion shall have an immediate correction date. 2. This policy will be submitted to the Northeast regional office for approval and will be reviewed with all staff once approved by the Northeast regional office. Staff will sign acknowledgement of the updated policy which will be kept in their staff files. The operator shall provide a correction date for when this portion of the plan will be completed.

Description: Date fee to be paid/Arrival/departure times

Noncompliance Area: During a renewal inspection on 10/22/25 Child 2 did not have the date which the fee is to be paid on their fee agreement. Child 3 did not have their arrival and departure times noted on their fee agreement.

Correction Required: An agreement shall specify the date on which the fee is to be paid. An agreement shall specify the child's arrival and departure times.

Description: Name/address/phone release person

Noncompliance Area: During a renewal inspection on 10/22/25 the emergency contact forms for Child 2 and 3 did not have addresses for all release persons. Child 3 also did not have the phone number listed for all release persons on their emergency contact form.

Correction Required: Emergency contact information must include the name, address and telephone number of the individual designated by the parent to whom the child may be released.

Description: Pertinent Laws & Regulations/General Health and Safety

Noncompliance Area: During a renewal inspection on 10/22/25 the facility did not have a written policy which addressed the identification of child maltreatment.

Correction Required: A facility shall be operated in conformity with applicable Federal and State laws and regulations. State agencies whose regulations may relate to the operation of a facility include the Department of Environmental Resources, the Department of Labor and Industry, the Department of Health, the Department of Education and the Department of Transportation. Conditions at the facility may not pose a threat to the health or safety of the children. The facility shall have a written policy which addresses: 1. Recognition of potential signs and symptoms of shaken baby syndrome and abusive head trauma 2. Strategies for coping with a crying, fussing, or distraught child; and 3. The prevention and identification of child maltreatment.

Regulation: 3270.151(a)

Description: 12 months prior to service and every 24 months thereafter

Noncompliance Area: During a renewal inspection on 10/22/25 Staff 1 had health assessments dated 6/14/23 and 10/7/25, which is more than 24 months between health assessments.

Correction Required: A facility person providing direct care who comes into contact with the children or who works with food preparation shall have a health assessment conducted within 12 months prior to providing initial service in a child care setting and every 24 months thereafter. A health assessment is valid for 24 months following the date of signature, if the person does not contract a communicable disease or develop a medical problem.

Regulation: 3270.37(b)(2)

Description: 8th grade & enrolled in training

Noncompliance Area: During a renewal inspection on 10/22/25 Staff 2 and 3 did not have documentation that the classroom portion of the their training curriculum was completed. Staff 2 and 3 also did not have verification that the training curriculum they were enrolled in met the requirements as described in §3270.31(c). Staff 2 and 3 had a letter from the representative of their secondary education institution which express the staff were currently enrolled in a early child education training program.

Correction Required: An aide shall have a minimum of an 8th grade education and enrollment in a training curriculum described at §3270.31(c). The classroom training portion of the curriculum shall be completed. Documentation of completion of classroom training and continuing enrollment in the training curriculum shall be included in the staff person's file.

Regulation: 3270.102(a)

Description: Clean and good repair

Noncompliance Area: During a renewal inspection on 1/16/25 cert rep observed a broken front piece on the wooden toy kitchen, in the front of Room 2/Main Room. The broken piece had a rough edge.

Correction Required: Toys, play equipment and other indoor and outdoor equipment used by the children shall be clean, in good repair and free from rough edges, sharp corners, pinch and crush points, splinters and exposed bolts.

Description: Clean, age appropriate

Noncompliance Area: During a renewal inspection on 1/16/25 cert rep observed that the rest mats available for use by the toddler children did not match the rest mat assignments. Toddler children were assigned mats 1-5, however only mats 1, 2, 4, 5 and 10 were present.

Correction Required: Individual, clean, age-appropriate rest equipment shall be provided for preschool, toddler and infant children as agreed between the child's parent and the operator. The rest equipment shall be labeled for the use of a specific child and used only by the specified child.

Regulation: 3270.124(b)(3)

Description: Parent home/work address, phone

Noncompliance Area: During a renewal inspection on 1/16/25 Cert rep noted Child 1 did not have a work phone number for the mother listed on the emergency contact form. Child 2 did not have work address and phone for the parents on the emergency contact form.

Correction Required: Emergency contact information must include the home and work addresses and telephone numbers of the enrolling parent.
