Caregivers did not document compliance with visually checking on sleeping infants aged 12 months or younger and/or the documents were not maintained for a minimum of one month. Upon checking the safe sleep charts for the children in attendance at 10:55AM, I found that it was documented that an infant fell asleep at 10:22, but checks were not documented after the initial time.

Children were not adequately supervised at all times. I reviewed camera footage for September 15, 2025, through September 23, 2025, and there were lapses of supervision during diaper changes as the staff member was unaware of the children’s activities. Children were seen standing on classroom furniture, biting others, and hitting others.

Diapers were not changed whenever they were soiled or wet and/or were changed on a shift basis. Documented diaper changes were reviewed for children in space #3 for September 23, 2025, and it was found that the children had a documented diaper change that occurred at 9:45AM. There were no other documented times of diaper changes.

Incident reports were not completed each time a child was injured, it did not include all the information required in rule, it was not signed by the parent and/or it was not maintained in the child's file. An incident report was not completed for the child that was injured on September 23, 2025. Incident reports on file for September 2025 were missing the facility id number, the time the parent is notified of the incident and by whom, and steps taken to prevent reoccurrence. Several of the incident reports on file were not completed with children’s last names, times of incidents, and dates of incidents.

A child was handled in a rough way, including shaking, pushing, shoving, pinching, slapping, biting, kicking, or spanking. On September 23, 2025, a staff member in space #3 was observed pulling a child by the upper arm to walk to their cot and then pushed the child to sit on the cot several times.

I have used this provider for less than 6 months

Most of the staff you deal with at the door except Emily are rude and unprofessional. The one who's in charge of billing who is Administrator now and former Director (Can't keep up with title) always tried to smooth things over which is okay until the point where acting like my son in a diaper for 9 hours straight was OKAY. She quickly let me withdraw them that day. I was also under the impression that they would feed my 2 and 3 year old breakfast as it was discussed with her and director then when I saw a teacher report (hardly get them) I quickly saw he wasn't being fed. I'm also still waiting for my 3 year old's underwear and socks to reappear as they didn't pack it up and Tina is trying to say he didn't have any... not one pair of backup underwear in cubby. The safety was a concern at one point on second week of August as a man was standing next to the drive lane to pick up and drop off. This man jumped on my truck trying to get a ride. He successfully jumped in the next parent's car in line and put it in drive when she stepped out to ring the bell to get her child. He was standing there the whole time they were opening door and letting kids out. So my assumption was absolutely no awareness of safety of releasing kids or surroundings. To make things even shady "Ms D" at the door was mad and unprofessional when I went to come in to bring my 2 year old in for his first day of school. I don't know anyone else who's okay with dropping off a kid without meeting the teacher and where child will be placed at. Since leaving I have shared our experience and have heard other stories from parents and glad we left. PS one of my kid's is on spectrum of autism...NOT handled well.

I have toured this provider's facility, but have not used its services

So me and my wife went to look at centers last we and came across this center in James City, I liked the location cause it was close to my job and with my son having autism I want to be close by in an emergency, I was assured that the staff was trained to handle children with multiple disabilities including autism. I stated my child was a (runner) and needed to be monitored to avoid him running off, I was assured that all doors had alarms and that could not happen. So my wife went to check on the center to look around and came home and said the center looked dated and just was not what we expected from a church daycare center and she felt staff could not handle a 8yo child with autism, we both were in agreement this center could not handle our child and was not up to par with modern centers in the area.
