Children were not adequately supervised at all times. In space #3, one caregiver was assisting children with toileting in the bathroom and the other caregiver was at the changing table diapering children. When the staff member at the changing table assisted the children in washing their hands at the sink, their back was turned away from the children. With one caregiver in the bathroom and the other caregiver facing away from the children, there were lapses of supervision.

Diapers were not changed whenever they were soiled or wet and/or were changed on a shift basis. In space #3, a child arrived at 7:00 am and was not changed until 9:34 am.

Children did not wash their hands upon arrival at the center, after each visit to the toilet, before eating, before and after water activity play, after outside play, and after handling animals or animal cages. In space #3, two children did not wash their hands upon arriving to the center and entering the classroom.

Staff did not wash their hands thoroughly before beginning work, before/after handling food, before bottle feeding or serving to other children, after toileting or handling body fluids, after diaper changing and after handling soiled items. In space #3, a staff member did not wash their hands after diapering a child.

Diaper changing surfaces were not cleaned with a detergent solution and disinfected after each use. In space #3, the diapering surface was not disinfected after being used to diaper a child.

Each child was not attended to in a nurturing and appropriate manner, or in keeping with the child's developmental needs. A staff member assigned to space #3 was seen telling a child crying due to hitting their head on the floor “no more crying” and “no more tears”. The staff member failed to provide an ice pack for the child’s head injury.
