All equipment and furnishings were not in good repair. On the outdoor play area there was a crack in the corner of the black plastic border surrounding the gravel pit. There was a brown and green plastic kitchen set that had a cracked door and a large crack in the inside of the play freezer.

Leftover medicines were not returned to the parent after the course of treatment was completed, after authorization was withdrawn or after authorization had expired and/or medication was not discarded within 72 hours of completion of treatment or withdrawal of authorization. In space 2, there were two (2) medications that the permission to administer had expired, one in February 2023 and one in March 2023.

Health and safety training topics were not included as part of on-going training within five years of completing the previous health and safety training topics. One of the staff had completed the health and safety training topics May 2017. Not all of the topics have been completed as of today's date.

All equipment and furnishings were not in good repair. There is one bolt, at the gravel pit, that is sticking up an inch. The steering wheel at the sandbox is loose causing a pinching hazard.

All corrosive agents, pesticides, bleaches, detergents, cleansers, polishes, any product which is under pressure in an aerosol dispenser, and any substance which may be hazardous to a child if ingested, inhaled, or handled were not stored in a locked room or cabinet. There are two (2) containers of lysol wipes located on an open shelf.

On or before the first day of work, all staff, including the director and individuals who volunteer more than once per week did not provide results indicating that they were free of active TB and/or TB test or screening was older than 12 months. One new employee, hired date January 24, 2022, did not have results until February 8, 2022.

Prior to employment a Criminal Background Check was not completed. An employee's hire date is January 24, 2022, but did not receive the qualifying letter until March 11, 2022.
