Diaper changing surfaces were not cleaned with a detergent solution and disinfected after each use. In space 1, the teacher used a sanitizer instead of a disinfectant. In space 5, a teacher only used a detergent solution.

A safe indoor and outdoor environment was not provided for the children. On the outdoor play area for one and two year old children there was gravel present. This is a choking hazard. On the outdoor play area for two year olds and older there were two (2) toys that have steering wheels that were loose and creating a pinching hazard.

While reviewing a video replay on August 3, 2023, I observed a teacher sitting at the children's table or in a camp outdoor chair and not moving throughout the indoor space and not interacting with the children as they were playing in activity areas.
