Each child's diapers were not changed at areas designated exclusively for diapering, on a surface that was smooth, nonabsorbent, easily cleanable and of tight construction. Per the teacher report for three children who wear pull-ups in the space used by the two year old's, a changing mat is not available nor is one used during diaper changes.

All corrosive agents, pesticides, bleaches, detergents, cleansers, polishes, any product which is under pressure in an aerosol dispenser, and any substance which may be hazardous to a child if ingested, inhaled, or handled were not stored in a locked room or cabinet. There was an aerosol can of Air Horne left on top of the children's cubbies and a can of room spray in an unlocked cabinet. Also a container of weed and grass pesticide was in an unlocked hallway storage closet.

Parent's medication authorization did not include required information. Medication authorization for a child whose date of birth was 9/4/2017 did not have the parent's signature.

Incident reports were not completed each time a child was injured, it did not include all the information required in rule, it was not signed by the parent and/or it was not maintained in the child's file. Incident reports dated 9/8/2022 for two children in the class used by the ones did not have the steps to prevent reoccurrence listed or how and when the parent was notified.

Center staff did not comply with the safe sleep policy. In the space used by the infants, at 10:10am a five month old whose date of birth was 3/20/2022, was observed asleep in the crib and the last documentation was recorded at 9:30am.

Child care providers and uncompensated providers who are not substitute providers or volunteers, including the director did not have a medical report on file prior to employment that was signed by a health care professional and/ or the medical report was older than 12 months. A medical reports for two staff members who began employment on 3/2/2022 and 1/18/2022 were dated for 3/3/2022 and 1/19/2022.

On or before the first day of work, all staff, including the director and individuals who volunteer more than once per week did not provide results indicating that they were free of active TB and/or TB test or screening was older than 12 months. A staff member who began employment on 8/1/2022, had a TB result on file dated 8/3/2022.

Staff did not model appropriate eating behaviors by consuming food or beverages that meet the nutritional requirements specified in the Meal Patterns for Children in Child Care Programs in the presence of children in care. An open can of Mt. Dew was left on top of the cubby in the space used by the children in the KPAII preschool class.

Leftover medicines were not returned to the parent after the course of treatment was completed, after authorization was withdrawn or after authorization had expired and/or medication was not discarded within 72 hours of completion of treatment or withdrawal of authorization. Per the assistant director's report, leftover medication for a child who disenrolled was not returned to the parent. The medication can not be located.

Medication authorization, giving the caregiver standing authorization did not meet the specifications in rule. Three children in the classroom for the two year old's had medication authorizations that were expired was prior to today's visit.
