A safe indoor and outdoor environment was not provided for the children. Two(2) screws were in an unlocked drawer in Space 135. These were removed during the visit. Fourteen(14) landscaping bolts were protruding from the landscape edging.

Electrical outlets and power strips, not in use, which were located in space used by children did not have safety outlets or were not covered with safety plugs unless located behind furniture or equipment that cannot be moved by a child. Four outlets in the gym were not covered with safety plugs.

Daily records of arrival and departure times for children enrolled at the center were not maintained as children arrive and depart and/or were not made available for review. Times were not documented showing when children departed yesterday and the time they arrived this morning in the classroom for 4 and 5 year old children.

Meals/snacks did not comply with the Meal Patterns for Children in Child Care Programs. Snacks brought from home did not meet the required meal pattern and were not supplemented. The director reported that parents have not provided opt out forms.

All stationary equipment, more than 18 inches high, was not installed over protective surfacing. Mulch measured three and four inches beneath the climbing structure on the playground for preschool children instead of the required six inches.

Signage was not posted regarding the smoking and tobacco restriction at the entrance of the center and/or in vehicles used to transport children. No smoking signage is not posted at the entrance where parents drop off and pick up children, and is not posted in vehicles used to transport children for field trips.

The operator did not notify the parent of each child enrolled in writing of the smoking and tobacco restriction. Signed statements were not on file verifying parents were notified in writing of the facility's tobacco restriction.

Each staff member did not have the required medical report, proof of tuberculosis test or screening and/or completed health questionnaire in a medical file, maintained separately from the staff member's individual personnel file. Health records for staff were filed in each staff's main personnel file and not separate as required.
