Initial Comments: The following deficiencies were observed at the time of the Complaint #159802 investigation conducted on 3/2/2026, and are subject to changes pending programmatic review. The Compliance Officer spoke with the Complainant on 3/2/2026. A full inspection was not conducted. The following room ratios were observed: One's/Two's: 1:6 Three's: 1:3, 1:9 Four's: 1:2, 1:7, 1:11 Two staff members were interviewed during this investigation. 1 staff file was reviewed as part of the investigation. 1 of 1 Fingerprint clearance cards reviewed were valid via a DPS website search. The following documentation was reviewed: Emergency cards. Upon completion of the Complaint investigation, it was determined from observation and staff interview that 1 of 1 allegations lacked sufficient evidence to be substantiated. The following citations were observed.

Initial Comments: There were no deficiencies observed at the time of the Initial Monitoring Inspection conducted on 02/17/2026 and are subject to changes pending programmatic review. A paper copy of the Notice of Inspection Rights was provided at the time of the inspection. A full inspection was not conducted at this time. During the exit interview, the following items were discussed but are not limited to: *Operating hours/School Calendar *Maximizing facility capacity
INSP-0165569 2025-12-30 Initial Monitoring Complete
Initial Comments: The Initial Monitoring Inspection could not be completed on 12/30/2025 because the facility was not in operation. A follow-up inspection will be scheduled. Please update the facility's Additional Hours and Closures in the Licensing Management System Portal to reflect the days that the facility will be closed or operating outside of Monday-Friday 730am-5pm as noted in the current facility profile.
INSP-0163012 2025-11-07 Compliance (Initial) Complete
Initial Comments: There were no deficiencies observed at the time of the Initial Compliance Inspection conducted on 11/07/2025, and are subject to changes pending programmatic review. The Compliance Officer provided a paper copy of the Notice of Inspection Rights to the facility at the time of inspection. The fingerprint clearance cards for 9 of 9 staff members were verified to be valid through the DPS website at the time of the inspection. During the exit interview, the following was discussed, but not limited to: *Staff files will be updated when the license is released: New Start Date, Fingerprint Verification, DCS Central Registry, TB Attestation, and new 10-day training *Classroom refrigerators and freezers that contain items for children will include a thermometer. *The cracked electrical faceplate will be inaccessible to enrolled children *The exposed (holes from the previous toilet roll dispenser) sharp steel located on the Girl's toilet room partitions will be inaccessible to enrolled children *The tear-off bar (used for paper roll) located on the diaper changing surface will be removed, and holes will be caulked. *The raised bolts at the base of the outdoor activity area fence will be inaccessible to enrolled children.
