Findings: Based on staff interview and Surveyor's observation, in the 1-year-old classroom, there was a cup with a blue liquid in it on the the diaper changing table. In the diaper changing sink there was a black substance, the staff stated they wash there paint brushes in the sink.

In the 2-year-old bathroom, in the toilet in the 1st stall was clogged. In the 4-year-old bathroom, in the 2nd stall there was brown substance all over the back of the toilet and the toilet seat.
