Equipment and furnishings were not sturdy, stable and free of hazards. In space #5 there was a piece of equipment accessible to children and identified by staff as the writing/art area that was not sturdy. The piece of equipment was 34' in height and 30' in width. The equipment had three (3) shelves and rollers on the bottom which based on observations did not lock and rolled freely. A 24-ounce mason jar filled with water was on the top of the piece of equipment. The jar had been used for a Beta fish that had recently died. Based on staff interviews, a three (3) year old child climbed on the piece of equipment a total of three (3) times. The third time the child climbed on the equipment, the equipment fell over on the child. The child was heard crying and staff observed the right-side top corner of the shelf (as facing the shelf) was on top of the child's left leg. There was water and shattered glass everywhere from the mason jar. | |||
