Children between 12 and 24 months of age were grouped with children 3 years of age or older. I reviewed the Face-To-Name Transition Sheets documentation for staff/child ratios dated September 7, 2023, for Space #3 and Space #2ab. In Space #3, at 7:49am one child, one year of age was signed into the classroom from Space #2ab. The staff/child ratio in Space #3 for children in care was documented from 7:49am to 9:13am as one staff member with eleven children one to three years of age. Documentation reviewed from 7:49am to 9:13am recorded there was one child, one year of age, nine children, two years of age, and one child, three years of age. At 9:13am and 9:23am, two additional children, two years of age were signed into Space #3 as documented on the Face-To Name Transition Sheet. The staff/child ratio from 9:23am to 10:29am was documented on the Face-To Name Transition Sheet as one staff member with thirteen children, one to three years of age. Documentation reviewed from 9:13am to 10:29am was recorded as one child, one year of age, eleven children, two years of age, and one child, three years of age being cared for in Space #3. At 10:29am, four children, two years of age and one child three years of age, were moved from Space #3 to Space #4. The staff child ratio in Space #3 at 10:29am was documented as one staff member with one child one year of age and seven children, two years of age.

The activity plan was not designed to stimulate emotional and social, health and physical, approaches to play and learning, language development and communication, and cognitive development for each group of children in care. In Space #1, the activity plan posted did not meet the developmental needs of the seven infants and one child, one year of age enrolled in the classroom.
