Each employee's personnel file did not contain an annual staff evaluation and a staff development plan. One staff member employed on July 25, 2018, did not have available for review the annual staff development plan and the annual staff evaluation documents. The last completed staff development plan available for review was completed on November 30, 2021. The last completed staff evaluation document available for review was completed on January 4, 2022.
