Initial Comments: The following deficiencies were observed at the time of the Compliance Inspection conducted on 10/29/2025 and are subject to changes pending programmatic review. Please submit the Plan of Corrections via the LMS portal within 10 days of receipt of the Statement of Deficiencies. The Compliance Officer provided the facility with a paper copy of the Notice of Inspection Rights at the start of the inspection. The Emergency Disaster Contact Form was completed at the time of the inspection. The DES Contact Group size was in compliance at the time of the inspection. The fingerprint clearance cards for 3 of 3 staff members were verified to be valid through the DPS website at the time of the inspection. During the exit interview, the following items were discussed, but not limited to: *Discussed the new rule set and the location of the updated documents. *Discussed the medication form.

The fingerprint clearance cards for 3 of 3 staff members were verified to be valid through the DPS website at the time of the Inspection. The Compliance Officer is Sherri Pavlisick.

NOTE: Both the Head Start and the Kidspace Programs were inspected. Name of Compliance Officer: Jennifer Flicker The Written Documentation is required to be submitted through the Licensing Portal within 10 days of the receipt of the Statement of Deficiencies. The following was discussed: *Rule requirements pertaining to substitute staff files/floater staff files available on the premises when the staff is working at a site. There were 3 staff files reviewed. 3 of the 3 fingerprint clearance cards were verified to be valid through the DPS website. The Empower Survey link was sent to the Director during the inspection.
