Cactus Wren provides an effective K-6 educational foundation. All students are considered special and all staff and students uphold multicultural respect. Special needs programs address academic learning differences, emotional disabilities, speech and language disabilities, and gifted in both resource and self-contained settings. Art, music and PE are provided at all grade levels. We are committed to the belief that each student must be afforded the opportunity for maximized growth academically, socially, and in developing personal responsibility.

Initial Comments: **AMENDED 3/5/2026 and 3/25/2026** The following deficiencies were observed at the Compliance Inspection conducted on 3/4/26, and are subject to changes pending programmatic review. The Plan of Corrections is requested to be submitted in the LMS portal within 10 days of receipt. A copy of the Notice of Inspection Rights was provided at the time of the inspection. BCCL staff emailed the Empower Self-Evaluation Assessment link to the Provider. The following was discussed but not limited to: 1) Please submit a copy of the current State fire Marshal Inspection when obtained. 2 of 2 fingerprint clearance cards were verified to be valid through the DPS website during the time of the inspection.

Initial Comments: The following deficiency was found at the time of the Compliance Inspection conducted on 3/11/2025 and is subject to changes pending programmatic review. The Written Documentation is required to be submitted through the Licensing Portal within 10 days of the receipt of the Statement of Deficiencies. There were 2 staff files reviewed. 2 of the 2 fingerprint clearance cards were verified to be valid through the DPS website.

Initial Comments: The following deficiencies were observed at the time of the Compliance Inspection conducted on 4/10/2024 and are subject to changes pending programmatic review. Please submit the Plan of Corrections via the LMS portal within 10 days. 2 of 2 Fingerprint Clearance cards reviewed were valid via a DPS website search. The Empower self-evaluation was completed at the time of the inspection. The Emergency Disaster Plan update form was completed at the time of the inspection. The following was discussed but not limited to: 1. Staff files must contain both the front and back sides of the fingerprint clearance cards. 2. Caulking at the base of the toilet in the Girls' toilet room. 3. The temperature in the freezer. 4. Role modeling for children (tone, words, voice). Compliance Officer is Tricia Tartaglio
