Initial Comments: The following deficiencies were observed during the Initial Monitoring Inspection and Complaint #158004 Investigation conducted on 03/10/2026, and are subject to changes pending programmatic review. A full inspection was not conducted at this time. The Compliance Officer provided a copy of the Notice of Inspection Rights to the Facility Director at the time of the inspection. Ratios observed were: Infants: 1:3 1s/2s: 1:4 3s-School Age: 2:12 There were 3 staff interviewed during this investigation. The following documents were reviewed during this investigation: Attendance Records, Rosters, written staff statements, and incident/injury reports Upon completion of the complaint investigation #158004, it was determined from observation, interview, and documentation, that 1 of 1 allegation was unable to be substantiated due to a lack of sufficient evidence. The following deficiencies were observed and are subject to changes pending programmatic review. Please submit the Plan of Corrections via the LMS portal within 10 days of receipt of the Statement of Deficiencies. During the exit interview, the following items were discussed, but are not limited to: *Hot water (86°F-110°F) will be accessible for handwashing at the diaper sink. The hot water valve was in the off position at the time of the inspection. *The torn laminate and exposed pressed wood on the art sink in the Young Achievers will be repaired or inaccessible if children are present. *Supervision means: for an enrolled child, knowledge of and accountability for the actions and whereabouts of the enrolled child, including the ability to see or hear the enrolled child at all times, to interact with the enrolled child, and to provide guidance to the enrolled child.

Initial Comments: The following deficiencies were observed at the time of the Initial inspection conducted on 9/18/2025, and are subject to changes pending programmatic review. 15 staff files were reviewed, and all fingerprint clearance cards were verified as valid via the DPS website. A paper copy of the Notice of Inspection Rights was provided to the Licensee during the inspection. The following was discussed, but not limited to: * Registration of Empower and completion of Emergency Disaster Preparedness forms following initial licensure, * Completing all staff file requirements following initial licensure, * Ensuring playground equipment is maintained in a clean condition, * Ensuring seats in the facility's motor vehicle are maintained free from hazards, * Ensure procedures are followed when diapering children with special health care needs, * Completing all required documentation on infant feeding instructions, * Posting of all allergies and/or modified diets in each activity area and the kitchen, * Ensuring soiled clothing containers are closed with a tight-fitting lid, * Ensuring ceiling tiles are secured to the ceiling, * Ensuring that facility maintenance is not completed around enrolled children, * Ensuring that support is provided in the Infant Room to maintain compliance with non-mobile infants. Please submit the following: * Written plan of corrections via the licensing portal, * Photos/documentation of corrections. NOTE: Licensure is pending the above requirements.
