Initial Comments: The following deficiencies were observed at the time of the Compliance Inspection conducted on 5/5/2025 and are subject to changes pending programmatic review. Please submit the Plan of Corrections via the LMS portal within 10 days of receipt of the Statement of Deficiencies. The Compliance Officer provided the facility with a paper copy of the Notice of Inspection Rights at the start of the inspection. The Emergency Disaster Contact Form was completed at the time of the inspection. The fingerprint clearance cards for 7 of 7 staff members were verified to be valid through the DPS website at the time of the inspection. During the exit interview, the following items were discussed, but not limited to: *Diaper changing sinks water must hold a temperature ranging from 86 degrees to 110 degrees.

Initial Comments: The purpose of the inspection was to conduct a Complaint Investigation (#00089384) on 8/29/2024. A full inspection was not conducted at this time. Please submit the Plan of Corrections via the LMS portal within 10 days of receipt of the Statement of Deficiencies. There were 2 staff interviewed during this investigation. There were 2 staff files reviewed during this investigation. Others interviewed: Director, Phoenix Police Officer, DCS agent. Documentation observed were 2 staff Incident Reports,1 Center Manager Incident Report, and 1 Director's Incident Report. Upon completion of Complaint Investigation #00089384, it was determined from observation, interviews, and documentation, that the allegations lacked sufficient evidence to be substantiated. The following deficiency was cited based on the investigation results and is subject to change pending programmatic review. Compliance Officer #1 is Sherri Pavlisick. Compliance Officer #2 is Jamie Anderson.

Initial Comments: The following deficiencies were observed at the time of the Compliance Inspection conducted on 05/09/2024 and are subject to changes pending programmatic review. Please submit the Plan of Corrections via the LMS portal within 10 days of receipt of the Statement of Deficiencies. The Emergency Disaster Contact form was completed at the time of the inspection. The Empower Self-Evaluation was completed at the time of the inspection. The DES Contact form was completed at the time of the inspection. The fingerprint clearance cards for 6 of 6 staff members were verified to be valid through the DPS website at the time of the inspection. During the exit interview, the following items were discussed but are not limited to: * Check water temperatures for the diaper changing sinks at both ends of the building often. *Follow through on finishing the blue cards completely.
