|Findings: Based on the Surveyor's observation and facility documentation, two of the ten rooms observed lacked the following: * Room #10 had 16 enrolled children physically present but only 14 marked in on the roster. * Room #3 had 6 enrolled children physically present but 8 marked in on the roster.|
