Findings: Findings include: Based on the Surveyor's observations and staff interview, a mesh baby gate was installed in the Infant Room's door to the playground, with the door open. Children 3-years-old through school-age were located on the playground, and two children from the playground were hanging over the gate interacting with the infants. The staff member stated she wanted the infants to have fresh air.
