|Findings: Findings include: Based on the Surveyor's observations and staff interview, a mesh baby gate was installed in the Infant Room's door to the playground, with the door open. Children 3-years-old through school-age were located on the playground, and two children from the playground were hanging over the gate interacting with the infants. The staff member stated she wanted the infants to have fresh air.|

There were 28 children present on the main playground and only 25 children were documented on the roster.|
|Findings: Based on facility documentation and the Surveyors' observations, it was determined that the facility did not contact DPS to verify the validity of the fingerprint clearance card for Staff #1.|
|Findings: Based on facility documentation and the Surveyors' observations, it was determined that 6 out of 15 enrolled children's Emergency Information, and Immunization Record cards reviewed lacked the following information: Child #1, Child #2, Child #3, Child #4, and Child #5: The date of the child's enrollment. Child #6: The contact telephone number for 2 individuals authorized by the child's parent to collect the child from the facility in case of emergency, or if the child's parent cannot be contacted.|
