Findings: Findings include: Based on facility documentation and staff interview, the file for Staff #1 lacked documentation that a fingerprint clearance card application was submitted to DPS within seven working days of employment. Staff #1 began employment on 10/18/17, and per a review of the DPS website on 12/27/17, an application was not received. The application in the file was dated 10/17/17. Staff #1 stated she received the application back from DPS, but she had not resubmitted the application. On 12/28/17 the Director sent the Department a copy of the return notice which was dated 10/25/17 and received by Staff #1 via email on the same date. Repeat deficiency from the 7/19/16 inspection.
