At our child care facility, your child’s education and well-being is our top priority. That’s why all of our teachers need at least six months of experience before hiring, and go through more training than state regulations require. In addition, each teacher must attend training meetings once per month to stay up-to-date with changes and trends in the child care industry.
Not only are our teachers qualified and experienced, but also our management team is equally experienced and works well with children and their parents. The friendly and courteous staff is knowledgeable and gladly goes out of its way to provide any assistance you or your children need.

|Findings: Surveyor reviewed 6 staff files and interview with the Director determined that 2 of the 6 staff files lacked documentation verifying that results of a negative Mantoux skin test given to a staff member was within 12 months before the starting date of employment or the staff member has had a positive Mantoux skin test or other tuberculosis screening test, lacked a written statement that the staff member is free from infectious active tuberculosis that is signed and dated by a health care provider within six months before the starting date of employment or volunteer service. * Staff #1-starting date of employment-6/1/2018. The file lacked documentation indicating that a negative Mantoux skin test was read The result was not verified before the starting date of employment. * Staff #2-starting date of employment-12/4/2017. The file had documentation indicating that the staff member was free from infectious active tuberculosis on 8/17/2015. The results were verified more than 6 months prior to the starting date of employment.|
|Findings: During a review of the Facility's Statement of Child Care Services and Surveyor observation, Surveyor determined that the Statement of Services did not include 1 of the 18 required elements of Child Care Services below: ** The procedures for notifying a parent at least 48 hours before a pesticide is applied on a facility ' s premises|
|Findings: Based on facility documentation, interview with the Director, and Surveyor observation, Surveyor was able to determine that a current fire inspection from a local fire department was not available for the facility. The last Fire Inspection expired 3/1/2018.|
|Findings: Based on a review of 6 staff records and interview with the Director at the time of the inspection, Surveyor was able to determine that staff #1 (starting date of employment-6/1/2018) lacked the staff member's written statement attesting to current immunity against measles, rubella, diphtheria, mumps, and pertussis.|
|Findings: The Surveyor observed and after testing with a thermometer determined that the running water at the diaper changing area sink in the 2's Room was 83 degrees Fahrenheit.|
|Findings: During an inspection of the facility, Surveyor observed the following item: 1) The ceiling vents had a white fluffy substance on the grill area in the School Age bathroom.|
|Findings: Based on a review of the medication permission slip for Child #1, Surveyor observed that the slip lacked the start and end dates (instructions for administration) for the medication.|
|Findings: During an inspection of the facility, it was found that the 2's Room had two unused electrical outlet on the east and west wall which lacked safety plug covers.|
|Findings: Based on observation, a telephone interview with Willie with Protection One Company and interview with the Director, Surveyor determined that the fire extinguishers in the facility were not tagged properly and could not be verified as being checked in the last 12 months.|
|Findings: Based on Surveyor observation, and a discussion with Staff #1, it was determined the emergency plan posted was dated 1/22/16.|
