The written authorization form for medication #2/child #2 lacked the starting and ending dates of the dosage period, the times/frequency of administration, and the reason for the medication. The written authorization form for medication #3/child #3 lacked a prescription number.|
|Findings: Based on the Surveyor's observations, it was determined that the following hazards were observed in licensed space accessible to enrolled children: * The electrical cord for the light table was laying loosely on the floor near the south wall in classroom #6. * There were several electrical cords laying loosely on the floor under the computer table near the west wall.|
