Findings: During the inspection of the infant room, the Surveyor and Team Leader observed a mini trampoline. The Director said that it was not being used as a trampoline but can be flipped over and used as another product, but she did not have documentation to show what the other item it could be used for. The Director was advised that trampolines are not allowed for use other than part of an individualized plan for a child with special needs.

Findings: During the inspection of the bathroom in the preschool room, the Surveyor observed that there was a toilet brush in the far corner behind a toilet seat that was accessible to children.

Findings: During the inspection of the diaper changing area in the toddler room, the Surveyor observed that there were two tubes of diapering cream that were not labeled with the child's name.

Findings: * During the inspection of the diaper changing area in the infant room, the Surveyor observed a rag laying on the ledge of the sink, an orange block that was under the bar of soap at the sink, and a cloth that was draped over the cart that is used as part of the diapering area (next to the diapering sink). On top of the cart was a kids apron, a hat, and two plastic buckets. There was also a black nylon bag hanging from the paper towel dispenser above the diapering sink. * During the inspection of the diaper changing area in the toddler room, the Surveyor observed that there is a small shelf unit next to the diapering counter that had a bin full of Legos right up against the diapering area and that there was a pack of wipes placed on top of a bin full of Legos.

Findings: During the inspection of the diaper changing area in the infant room, the Surveyor observed that there was a bar of soap sitting on the diapering sink. The soap dispenser hanging on the wall was empty.

Findings: * During the inspection of the diaper changing area in the infant room, the Surveyor observed that it was missing a soiled clothes container. * During the inspection of the diaper changing area in the toddler room, the Surveyor observed that there were two soiled diapers in a mesh garbage can that had no lid on it. The garbage can is not suitable for soiled diapers.
