Findings: The Team Leader reviewed the field trip form for Flandrau. The field trip occurred on 10/2/18 from 9-1. Child #2 was check as present each time attendance was reviewed during the field trip. In the parent signature area it was marked, "not attending". According to the Director, Child #2 did not attend the field trip. The Team Leader informed the Director that the child should not be marked in if they were not present.

Findings: The Team Leader and Surveyor observed a wooden picnic table that had paint chipping and was splintering. The picnic table was located in the outdoor activity area where the children were playing.
