Findings: During the inspection of the infant room, the Surveyor requested to see the infant daily log sheets and was given a "Infant Weekly Log Book" form. The Surveyor observed that this log listed the names of four children on one form and it was not specific to each child and did not specify the activities that each infant participated in. The Surveyor was given a copy of the log for documentation.

Findings: The Team Leader reviewed four personnel files and observed the following: * Staff 1 had a fingerprint clearance card expire on 01/02/21. The DPS website shows that the new application was not received until 04/12/21 and a new card was issued on 04/15/21. * The Director stated that Staff 1 did not work much during Covid. The Team Leader reviewed the staff time cards and observed that Staff 1 did work the first two weeks of April prior to the new card being issued.

Findings: The Team Leader reviewed the personnel file for Staff 2 and observed that she does not meet the minimum qualifications of a teacher-caregiver. Staff 2 was observed alone with children.

Findings: The Team Leader reviewed the children's emergency cards and observed the following: * Child 1 was missing the date of enrollment; the phone numbers to the two emergency contact names provided; the name and phone number to a health care provider; the name of the first person to be contacted in case of injury or sudden illness of the child. * Child 2 was missing the name and phone number to at least one additional emergency contact; the name of the first person to be contacted in case of injury or sudden illness of the child. * Child 3 was missing the name and phone number to at least one additional emergency contact; the name of the first person to be contacted in case of injury or sudden illness of the child. * Child 4 was missing the name and phone number to at least one additional emergency contact; the name of the first person to be contacted in case of injury or sudden illness of the child.

Findings: The Team Leader observed that the licensee did not ensure that a gas inspection was conducted every 12 months as the previous gas inspection was dated 02/2020 and the most current gas inspection was dated 06/25/2021.

Findings: The Team Leader reviewed four personnel files and observed the following: * Staff 2 had a diploma that was in Spanish but did not have documentation to show it was equivalent to at least a minimum of a US high school diploma. Staff 2 was observed alone with children.

Findings: During the inspection of the bathroom in Room B, the Surveyor observed that there was a plunger in one of the stalls that was accessible to enrolled children.

Findings: During the inspection of Room B, the Surveyor was unable to find a posted lesson plan. The staff member in the room acknowledged that there was no lesson plan.

Findings: During the inspection of the diaper changing area in Room C, the Surveyor observed that there was some toys on the diaper changing surface.

Findings: * During the inspection of the diaper changing area in Room C, the Surveyor observed that the roll of paper towels was sitting on the diapering counter and not in the dispenser. * During the inspection of the diaper changing area in Room D, the Surveyor observed that there was a roll of paper towels on top of the diaper changing surface. The staff said that the dispenser was currently not working and was not able to put the paper towels in it at the moment.

Findings: The Surveyor reviewed the main posting board area and observed that the posted menu was for the previous month (June). The staff member acknowledged that she had the current menu but forgot to switch them out.

Findings: Upon entrance to the facility, the Team Leader and Surveyor observed that there was a strong sewer gas odor at the entrance of the facility. The Surveyor observed that the odor was coming from the bathroom area in Room D.

Findings: The Surveyor inspected the first-aid kit and observed that it was missing Sterile gauze rolls.

I have used this provider for more than 6 months

The director and staff was notified that the staff kept putting other kids diapers on my kids who were in pullups next thing I was told my kids couldn't come back until they were tested for ADHD and they are2yearold which they can't e

I have used this provider for more than 6 months

The routine and schedules for the kids are amazing. My 3 kids love going. The arts and crafts,help with homework, and snacks help My littles maintain a healthy, productive day.
