| Initial Comments: There were no deficiencies observed at the time of the Compliance Inspection conducted on 3/30/26 and are subject to changes pending programmatic review. A copy of the Notice of Inspection Rights was provided at the time of the inspection. BCCL staff emailed the Empower Self-Evaluation Assessment link to the Provider. The fingerprint clearance cards for 2 of 2 staff members were verified to be valid through the DPS website at the time of the inspection. | Initial Comments: The following deficiencies were found at the time of the annual Compliance Inspection conducted on April 11, 2024, subject to changes pending programmatic review. A full inspection was conducted at this time. Two of two fingerprint clearance cards were verified to be valid through the DPS website during the time of the inspection. Inspection Item Expiration Dates: Insurance: 01/01/25 Fire: 07/25/24 Gas: 07/26/24 Items discussed, but not limited to: -Possibly lowering license tier before July 2024 renewal -Put N/A on child emergency card if second parent/guardian is not listed -Staff must have valid fingerprint clearance card before working with children -Updated Entrance Letter and DCS Direct Service form -Forms and rule set available on AZDHS website -Licenses are now valid for 1 year -Compliance Officer caseload change June 2024 The Plan of Correction was not accepted at the time of the inspection.
