| Initial Comments: The following deficiencies were found at the time of the Initial Licensure Inspection conducted 08/24/2026, subject to changes pending programmatic review. The Compliance Officers provided the facility with a copy of the Notice of Inspection Rights at the start of the inspection. 8 of 8 fingerprint clearance cards were verified to be valid through the DPS website during the time of the inspection. The facility was not licensed at the time of the inspection. Items discussed, but not limited to, were as follows: -Closure application for existing license/ownership -Fingerprinting and background check procedures -Maintain seamless surface in diaper changing areas -Classroom capacities -Keep pathways clear near exits -Signing in and out for children and staff -Children’s Emergency, Information, and Immunization Record card updated -Staff: files, start date, and training (10-Day and Annual) -Discipline and guidance procedures -Mandated Reporting of suspected abuse and neglect -Medication (form, storage, labels, rescue meds, etc.) -Supervision (nap, outside, and inside) -Fire safety: Extinguishers renewed yearly and fire/emergency drills monthly Please complete the Plan of Correction via the LMS Portal within 10 days of receipt of this Statement of Deficiencies. | |||
