|Findings: Surveyor 2 reviewed the field trip form for 10/23/18 to Apple Annie's and observed that the attendance was not marked during hourly (hour 1 and hour 2), when leaving the field trip destination and upon return to the facility at the end of the trip.|
|Findings: Surveyor 2 inspected the outdoor activity area and measured the sand at approximately 1.0 inches under the larger green slide and yellow slide; 2.0 under the green slide, and 3.0 under the blue slide.|
|Findings: Surveyor 2 reviewed two personnel files and observed the following: * Staff 1 had a negative TB test result dated 09/08/16 (start date 08/03/16).|
|Findings: Surveyor 1 reviewed the parent notice board in the three classrooms and observed that they were missing a chain of command.|
|Findings: Surveyor 2 reviewed 15 children ' s emergency cards and observed the following: * Child 1 was missing the name and phone number to one additional emergency contact. * Child 2 was missing the phone number to the health care provider.|
|Findings: a) Surveyor 1 reviewed the sign-in and out logs in Room #304 and observed that Child 3, Child 4, Child 5, and Child 6 were all signed-in and out throughout the month of April with a first initial and last name only. b) Surveyor 1 reviewed the sign-in and out logs in Room #300 and observed the following: * Child 7 and Child 8 were present in the room but not signed-in for the day. * Child 9 was signed-in for the next day (04/17) * Child 10 was not present but was signed-in for the day (04/16). * Child 11 did not have time-in or out recorded throughout the month of April. * Child 12 was being signed-in and out with a first initial and last name only throughout the month of April.|
|Findings: During the inspection of Room #300, the Surveyor was told that the sign-in and out logs are used as the roster as they take the binder with them when they leave the room. The Surveyor observed that the logs were not maintained as there were children physically present in the room but not signed-in which makes the roster inaccurate.|
|Findings: Surveyor 2 reviewed two personnel files and observed the following: * The Director had 16.5 hours of documented training between 08/2016 - 08/2017 (start date 08/04/10). * Staff 1 had 14.5 hours of documented training between 08/2016 - 08/2017 (start date 08/03/16).|
