Potentially hazardous items including but not limited to power tools, nails, chemicals, propane stoves, lawn mowers, gasoline, or kerosene were not stored in locked areas, removed from the premises, or made inaccessible to children. In space 1, an aerosol bug spray and a container of vaseline were stored in an unlocked cabinet drawer, less than five feet above ground and accessible to children in care.

Two staff members, hired on 6/19/2023 and 3/27/2023 did not have a medical on file.

School-age children were allowed to watch children's movies for one and half hour increments, three times per week.
