A safe indoor and outdoor environment was not provided for the children. On June 20, 2025, after sustaining serious head trauma and following the onset of symptoms, staff members placed a four-month-old child at additional risk of harm when the child was clearly in acute distress and staff failed to contact 911 for emergency medical services, which delayed diagnosis and treatment of the child.
