All walls and ceilings including doors and windows were not kept clean and in good repair. In space #1, #2, #3, #4, #5, and #6, paint is chipping in doors, and walls. Also in space #5, the cabinet where the handwashing sink is has paint chipping.
2023-11-07 Violation 840 .2820(b)
All corrosive agents, pesticides, bleaches, detergents, cleansers, polishes, any product which is under pressure in an aerosol dispenser, and any substance which may be hazardous to a child if ingested, inhaled, or handled were not stored in a locked room or cabinet. In space #4, a bottle of floor solution in a spray bottle was sitting on a paper towel holder. This was corrected by the administrator locking the solution in a locked cabinet.

All staff did not successfully complete certification in First Aid appropriate to the age of children in care. Verification of staff completion of First Aid training from an approved training organization was not in the staff file. One staff had not completed First Aid training or was not in staff file for review with-in 90days of hire. Refer to staff and training worksheet. This is a repeat violation from visit completed 11/21/2022.

All staff did not successfully complete certification in CPR training appropriate to the age of the children in care. Verification of staff completion of the CPR course from an approved training organization was not in the staff file. One staff had not completed CPR training or training was not on file for review with-in 90 days of hire. Refer to staff and training worksheet. This is a repeat violation from visit completed on 11/21/2022.

Each new employee did not complete, within the first two weeks of employment, six clock hours of training in required topic areas. Three staff had not completed the first two weeks , six clock hours of training in required topic area of the orientation form. Refer to staff and training worksheet. The Prevention of Shaken Baby Syndrome and Abusive Head Trauma policy was not reviewed with new staff prior to providing care with children and/or a signed acknowledgement with all the required information was not maintained in the staff person's file. Three staff had not completed the Shaken Baby Syndrome and Abusive Head Trauma policy prior to providing care with children. However the policy was reviewed during todays visit and signed by all three staff. refer to staff and training worksheet. This is a repeat violation from visit completed on 11/21/2022.
