In the outdoor area where preschool children play, there were brown leaves were accumulated against the building. A play structure had chipped climbing pegs with point edges, and the black border trim was separated and one screw was exposed.

Operator has not scheduled and obtained a fire inspection within 12 months of the previous inspection. Operator did not submit the original approved report to DCDEE within one week of the inspection visit on a form provided by the Division. The fire inspection dated for 2/17/2022 was not submitted within one week of inspection visit. It was submitted on 3/1/2022.

A current activity plan was not posted for each group of children for reference. The activity plan in space #3, were not current. The provider place and updated activity plan in the classroom to reflect 11/7-11/2022.

Equipment and furnishings were not sturdy, stable and free of hazards. In the bathroom of the hallway near space #2, the door to the cabinet is hanging from the hinges.

Fire drills were not practiced monthly and/or the drill record was incomplete. The fire drill log sheet had not been updated. The provider stated that she had forgotten to document the sheet. The provider documented the sheet during today's visit.

A safe indoor and outdoor environment was not provided for the children. In the outdoor play area on the steps leading to play area #1, the carpet on the steps was ripped to different places. In play area #1 of the outdoor space in the sand was some black netting under the sand that could be a chocking or tripping hazard for children. The hand rail leading out to the play are in both space #1 and #2 has exposed rust.

All corrosive agents, pesticides, bleaches, detergents, cleansers, polishes, any product which is under pressure in an aerosol dispenser, and any substance which may be hazardous to a child if ingested, inhaled, or handled were not stored in a locked room or cabinet. In the bathroom on top of the toilet in space #2, there was a package of Huggies wipes that had keep out of reach of children on the label. The teacher placed the package of wipes on a shelf in the classroom that was five feet from the floor.

Monthly playground inspections were not completed and/or they were not completed by an individual trained in playground safety requirements. The last playground inspection completed was dated for September 2022. The provider completed an inspection for October 2022.
