| The outdoor premises were not clean, drained and free of litter and hazardous materials grass and other vegetation in a manner which does not encourage vermin. There was several pieces of trash and 2 half filled plastic bags of sand laying around in the outdoor play area. The provider picked up the trash and the half filled bags and discarded during the visit. | |||

| A safe indoor and outdoor environment was not provided for the children. In the out door play area of the facility there where two spaces sand bags were placed to help with water flow and the removal of the stand, however there were holes were the sand was not replaced which could cause for the children to trip or fall. | |||
