All walls and ceilings including doors and windows were not kept clean and in good repair. In space#7, located in the block area there is a small hole in wall that is accessible to children in care. This was corrected during the visit, the provider moved a shelf to the cover the area to make inaccessible to children in care until repairs can be made.

On two different occasions March 15th and 23rd 2023, sanitation guidelines were not being followed, one staff member in space#3, placed the wrong nipple and screw ring on labeled bottles after cleaning to be returned home at the end of day.

One new staff member hired June 27, 2022, did not have a signed acknowledgement of The Prevention of Shaken Baby Syndrome and Abusive Head Trauma policy on file during today's visit.

All corrosive agents, pesticides, bleaches, detergents, cleansers, polishes, any product which is under pressure in an aerosol dispenser, and any substance which may be hazardous to a child if ingested, inhaled, or handled were not stored in a locked room or cabinet. Located in space #4, on the counter near the restroom, accessible to children under the age of 3 were a container of clear liquid hand soap labeled keep out of reach of children.

Plastic bags, materials that could be torn apart and toy parts small enough to be swallowed were accessible to children under three years of age. Located on the changing table in space#2, plastic gloves were accessible to children under three years of age.
