The operator made an effort to falsify information.On February 22, 2022, the violation regarding staff Gloria Hacket-Smith not having a current Criminal Background Check Qualifying Letter was observed and documented. On March 9, 2022, a compliance letter was received via email from operator Ms. Evans stated “Employee G. Hackett-Smith is not able to work until qualified letter is on file.” On March 10, 2022, I checked the regulatory system and Gloria Hacket-Smith, employee without Criminal Background Check Qualifying Letter during the Annual Compliance Visit completed February 22, 2022, had not been qualified. During today’s visit, Gloria Hacket-Smith was present and working at the facility.
